| Question: How can I know whether I have diabetics or not without getting the blood test done. | Name: ELIZABETH |
| Answer: Sir,
Not possible. You have to test blood glucose to find out whether you are diabetic or not.Symptoms of diabetes occur very late and can be of some other disease too. There are some instruments which measure glucose without pricking the skin, used in diabetic subjects for detailed evaluation.
Glucowatch is a device worn like a watch. It has limitations using in countries like in India because of hot climate since patients can sweat and interfere with measurement of glucose from the skin.Another device is CGMS: visit-http://www.jothydev.net/html/cgms.htm Both require use of glucometer and finger prick for calibration!!! | Email: emathew@cmai.org |
| . |
| Question: do you name any instrument to check the blood sugar without sticking the finger? | Name: bency.achenkunju |

| Question: i am taking 4 daonil every day to maintain my sugar level. what type of tablet to take for avoiding the side effect . such [acidity . gas trouble etc] | Name: RAJI |
| Answer: Sir,
The best method of cure in diabetes is to diagnose it at the stage of early pre diabetes(fasting sugar 100mg/dl and less than 125mg%) and then cure it completely with diet, exercise and drugs if required.Glibenclamide is better avoided in the elderly and those with risk factors for heart disease. If diabetes is treated properly there is nothing to fear about it.

| Question: Dear Doctor,My fasting Blood Sugar is about 130-140. But random Blood Sugar checking never have greater than 140. Do I have diabtes? If yes, can I control diabetes by adopting strict dietery practice and excercise? Also, is it true that the best method of controling diabetes is to start taking insiling as per doctor's advise whenever diabetes is detected?ReagardsPrakash | Name: Prakash |
| Answer: Sir,
First of all you should understand that diabetes is now more than just blood sugar. You have to undergo detailed physical and lab evaluation before proceeding. If fasting is more than 100 and 2 hr postprandial is 140 or more you have prediabetes. But since fasting is more than 126mg% you have to undergo testing after 2 hrs of 75 gm glucose intake.Initial treatment of diabetes whether with diet/exercise alone or with insulin has to be decided on several parameters and varies from individual to individual.

| Question: My blood sugar after fasting 125mgIs it required any medicine ?. | Name: baby |
| Answer: Sir,
We cannot take a decision based on a single fasting value. We need to do a battery of other tests.Anxiety during blood test can also raise sugar value in some cases but is only transient. Please repeat the test.A fasting more than or equal to 100mg% but below 126mg% is termed pre diabetes. Please consult your doctor to get advise on control of risk factors if present.

| Question: My name is reji, working in Saudi Arabia. Age 40 yrs. I am a BP patient and also having little asthma. No diabetic still now. What are the precautions to take to prevent diabetic for BP patient, and also rquest to give more about diabetic by this mail, please.reji, ksa | Name: reji |
| Answer: Sir,
Blood pressure is one risk factor for the development of diabetes. If above 140/90mmHg you should see your doctor again for dose adjustments. For Asian Indians if abdominal circumferance is more than 90cm in men and above 80cm in women it also increase chances for diabetes and heart attacks due to excess abdominal fat.If you are on oral steroids for asthma you are again at high risk.Always take only inhaler medications for asthma and never any oral medications.thanks.jothydev | Email: civil@al-babtain.com.sa |

| Question: Doctor,I am 58 years old, an x-tray technician. Diabetic since 5 years and also taking medicine for thyroid and pressure. By God's grace, still alive to look after my grandchildren. I don't sit idle at all, always busy with some work. But sometimes I have severe joint pains and pain inside the feet. One of my small finger is always in a numb stage, but even for a small touch, pains a lot. Is it all because of diabetics? I do not have cholesterol, but I have tryclicide. What is your suggestion for me to live longer?Looking forward to hearing from you. Thank you! | Name: Annamma |
| Answer: Dear Madam,
Good to know that you are leading a very active life. You are on medications for diabetes, hypothyroidism and Blood pressure; right? The numbness is due to neuropathy which is a consequence of poorly controlled diabetes. To 'live longer' alone should not be your objective : you should be capable of healthy ageing and living longer without any serious disability. For that pl make sure that your A1c is normal and your diabetes is taken care of by a trained diabetes team so that you receive a comprehensive care.

| Question: To control the food, I read in a web magazine that we should avoid the food that have 'carbohydrates', and I did so. But recently I heard a radio conversation and the Indian doctor advised that the carbohydrate not a problem, but we must avoid the food with 'fat'. While I am living in Gulf could you please advise what kind of food we can eat? Thanks,Biju | Name: Biju |
| Answer: Sir,
Carbohydrates are absolutely essential in the diet even for patients with diabetes since it is the energy source for the human body.May be 55% of the diet should have it.In case the blood sugar is high we have to have medications to control it along with intake of healthy food.Restriction on saturated fat intake will help in prevention of diabetes as well as treatment of diabetes. You can look at food labels to get an idea on this concept.jothydev | Email: biju@alliedarms.com |

| Question: Is it right fasting can prevent high shugar level ? | Name: joshythomas |
| Answer: Sir,
Need not be true always. You may come across very many patients who are obese and with diabetes who are used to frequent fasting.Another lot will be fat subjects who deliberately miss their supper or breakfast to lose weight. It is better to eat at the right time and do not miss your main meals but quantity of each meal should be kept optimal.Diabetes is the result of overeating and overweight.

Pleast advise me.Thanks,Gopalan | Name: K.V.GOPALAN |
| Answer: Sir,
It is a fact that 2 out of 3 diabetes patients sustain a heart attack. It is preventable once you control your sugars and BP and LDL according to recommendations. We need medications along with exercise and diet to achieve these goals. After a heart attack you may need insulin shots for some reason which you should discuss with your diabetes care team and decide soon.

| Question: I want to know what are the food I can eat and what are the food items should not eat. Kindly arrange to forward the list to me if possible please. | Name: P |
| Answer: Sir,
It was very recently that it got established that there is nothing called "diabetic diet".A prescription diet never works good, and over the years we have been experiencing the bitter results of that traditional practice. A diabetic patient can eat anything provided he knows what he is doing and sugar values remains below target levels. The modern rule of carb counting is the basis of this rule. Each pateint is different and the rule varies.15gm of carbohydrate(one carb) is common in one apple, 1 small banana, one orange, half mango, one iddli!! Pl meet your diabtic health care team for customising your requirements.

I will advise you to meet a diabetes health care team while planning for a baby.I presume that your husband is very young and hence please prioritise. If sugars are very high and continues to be high due to lack of attention, the chances are that he has to be on medications life long. The initial treatment of newly diagnosed diabetes is very critical in determining the future!!!

| Question: i finished my hernia operation 7 months before, now i start getting pain again like before. my job is secretary in dubai. i am diabetics and pressure patient taking tablets. is there any relatio between these problems. | Name: jose |
| Answer: Sir,
Infection/ diabetes is like a vicious circle. If your sugars are high the chances of infection goes up.It is found that modification of dosages of medications for sugar and BP may be required at times of stress. If your sugar values are fluctuating , then there may be some link between that and your recurrence of pain.
Pl contact your diabetologist first and then your surgeon.

| Question: I am a type 2 diabetic patient for 4 years and taking medicines for 1 year. I am also suffering from high colostrol and Blood pressure for 1 year. How can I control my dibetic? What is the diet I can take ? | Name: Suresh |
| Answer: Sir,
Treatment of diabetes includes treatment of sugar, cholesterol, Blood Pressure etc., It is now proven that treatment of Blood pressure in diabetes is more important than treating blood sugar. You have to make BP as normal as possible.Diet is based on diabetes status and presence and absence of other diseases. It is important that you should be maintaining normal lab values.Modern advise is to go for a healthy diet for the whole family and the concept of diabetes diet is disappearing.

The International recommendations for screening may not always help Asian Indians in early recognition of diabetes. We have to start sreening early and should be more frequent. You have to test fasting sugar once in 6 months. It has to be kept well below 100mg%.Eating fat will trigger diabetes. If you are eating more sugar it will also help diabetes onset by increasing body weight.Make sure you get amble time for daily exercise to keep away diabetes.

| Question: will the diabetes caused by medicines be completely cured? | Name: laly |
| Answer: Madam,
It is true that some drugs like steroids when taken orally or in the form of injections can induce diabetes. If detected and treated in the initial stages itself, it can disappear once drug is withdrawn. If high sugars persist for a longer time, then beta cells will be damaged altogether and insulin production stops. In some conditions of the skin, certain cancers, terminally ill pts etc., such medications have to be used inspite of the knowledge that they will produce diseases like diabetes.

There is nothing called 'minor' diabetes and 'severe' diabetes. If somebody is having diabetes it should be treated, with either diet and exercise with or without drugs as the condition warrants.Pl measure A1c and lipids too.Complications of higher than normal sugars can occur even with minimal elevations of sugar. You can measure your blood sugar 2 hrs after consumption of 75 gm anhydrous glucose and if it is more than 200mg% diabetes is confirmed.Take care. jothydev@vsnl.com | Email: jabirtirur@hotmail.com |

| Question: Dear Sir,I am a dibetic for the last 2 years. My PP Sugar (2 hours after food) is 187 when I checked my blood during the last month.Kindly let me know whether this level of sugar will damage my internal/external ( such as eye)organs at present.Let me know which level of bllod sugar will affect our organs, veins etc. | Name: Jayachandran.A.C |
| Answer: Sir,
In each patient the treating doctor will specify a target for blood glucose.Even minimal increases in blood sugar(say more than 140mg%) is now proven to damage internal organs and hence the introduction of newer intensive protocols. Moreover high sugars will ultimately damage the beta cells, making insulin injections mandatory in the treament for achieving normal sugar levels. The major reason for not aiming at normal sugars in ordinary treatment is because of fear of sugars being very low and becoming extremely dangerous.Low sugar is deadly but high sugars are slow and silent killers.Pl meet your diabetes care team.

| Answer: Sir,
Older generation drugs are very potent but produce insulin by stimulating beta cells. In type 2 diabetes if sufficient insulin is present we can use sensitisers so that hypo will not appear but at the same time beta cells will not become exhausted too.Pl meet your diabetes expert and hs team.

| Question: When you have diabetes for long time, it affects your right toe and I have heard stories about right toe amputaion to save further complications. Why does diabetes affect right toe first? | Name: Raj |
| Answer: Sir,
That is strange. It is true that in every 30 seconds a foot is lost due to diabetes. It is not true that diabetes first affects the right toe.Amputation usually starts with a toe and finally ends up at above knee if basic diabetes is not taken care of properly.Amputaion becomes a life saving procedure when it is feared that the pus from the feet will spread to other parts of body and patient will die of it.In modern diabetes treatment we have proven that these complications are all totally avoidable.

| Question: Is it true that the medicines taken regularly for diabetes may affect kidney badly and Is it possible to control this only through food control and daily exercise without taking medicine ? | Name: Sreeja |
| Answer: Madam,
Medications if properly selected and used according to instructions cannot damage organs but are used to prevent damage. For example medicines like ramipril, losartan etc., protect the kidneys but if not used properly will damage them.Diet and exercise should be the best to prevent diabetes: once the disease has occured your aim should be to maintain normal lab values whatever be the safe method of treatment advocated.

| Question: Hi Doctor, My mother is a diabetic patient for the last 15 years. She is 63 years old now. 3 months ago she undergone a surgery to remove a cyst on her neck. After that surgery her sugar level is hard to control even if she is taking dianil 3 times a day, and her ESR is 60. Please advise what is the reason for the sugar level to be high and what is the reason for the ESR to be so high, Thanks in advance. | Name: Smitha |
| Answer: Madam,
Good to see your concern over your mother.Stress whether it is physical or mental will increase blood sugars. The physical stress of surgery could have created the present problem.Patients who are on conventional treatment will ultimately require insulin shots to maintain normal sugars. Some latent infection could be the reason for high ESR which will also subside if she is properly managed,Pl meet your doctor.

| Question: Dear Doctorcan you tell me the reason why, Suger count is more in mroning then in the evening time ? | Name: JOBY |
| Answer: Sir,
In uncontrolled diabetes, blood sugar fluctuations are so scary that we have to have newer modalities of treatment to stabilise it round the clock.Pl visit http://www.jothydev.net/html/cgms.htm
In a subset of patients, liver produces glucose and even fasting sugar can be high in the early morning but later it comes down.

| Answer: Sir,
It is good to see that your are controlling your sugar through strict dietary control.I am not sure about your HbA1c level which is absolutely essential for you to determine the average levels.Diabetes is not just blood sugar control as seen in my answers in these web pages.Your burning sensation in the abdomen is due to possible peptic ulcer.Don't fast in such a way that you are becoming ill.Eat at the right time and eat adequately to maintain your weight and activity level. If your sugar rises we will treat it.Don't worry.

| Question: my mother is having diabetes for almost 25 years. Taking insulin daily and having kidney trouble and other complications. She does not like to take any medicine. Is there any way to avoid medicine?? | Name: Safeer |
| Answer: Sir,
If your mother is having diabetes with its complications, there is no doubt that she should be on the best medications.Diabetes is a major cause of disability in old age in which pt is dependent on others until death. There is a false belief among the elderly diabetic pts that medications are dangerous. They should be well educated on the severity of the complications and the discomfort of undergoing dialysis, enormous cost involved and living the rest of their lives blind and disabled.Investing on preventive diabetic care is the most cost effective method.

| Question: The last reading for me for sugar test was 158 after the food. Kindly let me know what is the reading says. | Name: Sony |
| Answer: Sir,
If you are a patient on treatment then the doctor will set targets of treatment for you based on severity of diabetes. For example if you are young(after 19 years) and without any complications then the blood sugar is better close to that of a normal person. If the pt is very old with lots of complications, then it can be a little bit on the higher side due to risks involved in hypoglycemia. If you are not a pt, pl see a doctor and conduct a complete work up.

| Answer: Dear Shaji,
Manorama web site will guide you on how to find out Body mass index. It is better to be less than 23kg/m2 for Asians.More important is control of abdominal obesity.Dieting is very very good.If your blood sugars are above 140mg%(average) then beta cells will be gradually destroyed via a phenomenon called glucotoxicity. This is very common in our pts and eventually they become dependant on insulin.Pl measure A1c too.Preserve your beta cell mass.Take care.

Your symptoms ar due to low sugar which requires modification in the dosage of Gliciphage which should be ideally done by your physician. You are on medications for diabetes and high blood pressure. These two illnesses are part of life and need close monitoring at regular intervals by an expert team of doctors.I have seen Malayalees working in the middle east getting their medications from Kerala and visiting the doctor once in two years or so.(We have thousands of patients working in the Middle east , but we follow them up from here via telephone or internet since 6 to 7 years). If you are not on follow up it can be dangerous.Pl take care.

| Question: what is the first stages charectaters and appearance of diesese | Name: Aboobacker |
| Answer: Diabetes can be divided into different stages based on duration and severity.Type 2 diabetes the commonest type of diabetes is asymptomatic for a long period say:8 to 10 years and hence early detection is possible only if you examine your blood.Complications like heart attack, or retinopathy need not occur late in the disease and can even occur in the early stages. The moral of the story is : Should not wait for diabetes or its complications to set in.Prevent it !!
